Complete and sign an ACH Authorization Form and include a voided check.

  • Vendor Number
  • Legal Name
  • Mailing Address
  • Invoice number(s) for missing payment
  • Amount of missing payment

Complete the appropriate signed form for address/bank changes. For ACH updates, include a voided check.

IRS 1099-MISC forms are mailed by January 31 each year, per IRS guidelines.

  • For mergers: Certificate of Merger and executed W-9
  • Otherwise: Certificate of Name Change and executed W-9
